Dazzle allows you to create intelligent data gathering questionnaires.

An example of use would be where admin persons set up the header information for a report, the questionnaire is then sent to a client to complete and a doctor or expert accesses opinion and conclusion sections after reviewing the whole questionnaire.

When the form is finalised for submission the data is linked to a report generator utility which creates a mailmerged output file in MS Word.

The report generation process takes seconds to complete.

Create a questionnaire

Dazzle allows you to create intelligent data gathering questionnaires.
The questionnaires produced are PC, Tablet and Mobile friendly.
You can create as many questionnaire templates as you like.

Send a link to the applicant/patient/client

Select your client and select which questionnaire you wish to dispatch.
The applicant will receive an email link which will open the questionnaire for completion.

Applicant completes the questionnaire and submits it

On completion of the questionnaire the form is submitted by the applicant.
Email notifications are sent to team members to inform them questionnaire is awaiting their input.

Special sections of the questionnaire can be accessed by authorised persons

Admin staff could have accessed the header part of the form to enter pertinent data.
The applicant is sent the form but does not have to see the already completed admin section or the sections where the reporting expert will have access to enter their opinion and conclusions.

Report is generated by mailmerging into MS Word

The information gathered in the questionnaire can then be transformed into a well-formed Microsoft Word document at the push of a button.
Dazzle handles all manner of complicated items such as gender inflections and sentence capitalisation. In other words, the engine is intuitive enough to correctly insert a him/her he/she his/hers etc. based on the gender of the subject of the document.
