The system is a software solution.

It will sit on your server allowing you to send emails out to your list-base inviting them to click on a link within the email inviting them to open up the questionnaire you want them to complete.

The recipient will receive an email with a link embedded into it. When the link is clicked the survey or questionnaire form will open.

Sections

The questionnaire can be divided into sections and certain sections and input fields can be isolated in order for somebody else to access protected sections and fields.

For instance, the questionnaire could be broken up into 'Admin only' entry sections, 'Consultant sections' where a medical consultant has ability to enter free form paragraph text to provide a opinion or conclusion and a 'Recipient/client' section where the main body of the questions are accessed.

Note:

  • The 'Consultant section' and all other sections can be viewed and nominated data entry fields can be accessed by the 'Consultant'.
  • The 'Recipient' does not get to see the admin or consultant sections of the questionnaire so cannot enter any data into them.
  • The 'Admin' people can see and access just their section if required or could have view access to the whole form if needed.

Conditional Input

Certain questions and sub-divisions of questions can be presented based on key question conditions.

Example:

Q: How long have you been driving? < Will not ask or display if answer to above question is 'No'>

List could be of two types: a tick list allowing you to select more than one item or a list allowing you to select one item only.

Report would be set up to automatically produce following output (assumes 1,3 and 5 selected from list):

The candidate stated that their next purchase of motor vehicle could be one of the following:

A Toyota, Audi or a Nissan.

Note - Our system facilitates commas between items selected and an end break as in 'or a' above.

System Overview

As described, the first part of the solution is a bespoke questionnaire creator, the end product of which is a questionnaire that can be sent to someone to collect data or taken with you to be used to gather information about a person, premises or anything you want to collate data on such as an inspection visit.

Once the data has been gathered it is collected online and stored in order that a report can be created automatically. The report can contain front sheets, images and header information and the output of which is generated by merging the survey data with Microsoft Word.

We provide a programmable interface that will allow you to connect this system into applications that you may have developed or you may ask  us to write you a bespoke application to manage your business data.

In addition we offer a managed service for clients that do not particularly wish to want to learn how to use the tools we provide to build questionnaires and reports and may prefer that we undertake this work on your behalf.

We refer to the whole process as document automation as it creates full comprehensive reports automatically and to the degree that if the questionnaire and reports are designed properly, you would not really know that limited human intervention was present in the making of the final report.

Overall Benefits

It's mainly time and money saving that benefits your business. In our example above, nobody visited the candidate to ask them to complete the questionnaire; it was sent as an email.

The expert, entered a couple of paragraphs of text but added large amounts of padding to the report in the form of pages of pre-defined information sections by ticking from a list within the questionnaire that said 'Include the following items:'

[   ] Audi engines,
[   ] Nissan engines

Note - All above items could be automatically included as standard based on conditional pre-selected items asked within a connected question using the systems logic engine.
